Teachers charged with a coordinating role play a substantial part in the quality of teaching delivered at the University, however their contribution is often behind the scenes, and thus not amenable to evaluation by students. In such cases, members of the teaching team (e.g. tutors/demonstrators) are in the best position to give feedback on the performance of the coordinator who leads them. The primary purpose of this questionnaire is to help with teaching development, which in this case includes coordination activities.
The questionnaire consists of between five and ten questions chosen from a bank of 20 questions on various aspects of coordination. The results may be submitted as evidence of your teaching with the Otago Teaching Profile.
How to order an evaluation
Please note that we need 5 working days’ notice to set up your questionnaire from when we receive your order (for both paper and online questionnaires).
You can order questionnaire forms online using Otago InForm, a new online system for ordering questionnaires quickly and easily. Log in using your University username and password, select which questionnaire type and start designing your questionnaire. Once you are happy with your questionnaire, proceed to checkout and submit your request. You will be sent a confirmation email with the details of your request for your records.
Print off an order form
If you prefer, you can still make your request by completing a paper request form – available for download on the forms and resources page – and returning it by fax, post or email. Further instructions, and the question catalogue, are included in the documents.
Questionnaire form delivery method
When you request your evaluation, you will need to specify whether you would like your teaching team to fill out paper or online questionnaire forms.
If using Otago Inform, you will be asked to select your chosen distribution method at the checkout stage.
If you lodge your questionnaire request by sending in a paper form, you will need to fill in a different form depending on the delivery method you choose. The two forms are labelled on the forms and resources page.
Confidentiality policy for coordinator/team leader evaluations
The results of your questionnaires will be confidential to you - HEDC will not supply anyone else with copies or identifiable information from the questionnaire without express authorisation from you to do so. Occasionally, the Evaluation Coordinator may need to confer with the HEDC Head of Department with regard to procedural matters. Your results will not be seen by any other HEDC staff, although they are available to assist you with the interpretation of your results if asked. How you use the information resulting from the survey is a matter for you to decide. However, if you are a candidate for promotion, confirmation or appraisal, you may choose to submit the summary report of results with your application or report (note: this is optional).
