With an individual teacher questionnaire, students are evaluating the named teacher. Students can provide valuable feedback about aspects of teaching such as:
- teaching strategies
- student learning
- professional attitudes
There are five compulsory questions, with five additional questions chosen from a catalogue of 45 questions. These questionnaires should also be used as part of preparing an Otago Teaching Profile for promotion, confirmation or other performance appraisal. The promotions policy states that you must submit student evaluation of teaching performance using the individual teacher instrument for all your significant teaching responsibilities for the past three years.
How to order a questionnaire
Please note that we need 5 working days’ notice to set up your questionnaire from when we receive your order (for both paper and online questionnaires).
You can order questionnaire forms online using Otago inFORM, a new web-based system for ordering questionnaires quickly and easily. Log in using your University username and password, select which questionnaire type and start designing your questionnaire. Once you are happy with your questionnaire, proceed to checkout and submit your request. You will be sent a confirmation email with the details of your request for your records.
Print off an order form
If you prefer, you can still make your request by completing a paper request form – available for download on the forms and resources page – and returning it by fax, post or email. Note that you will also need to refer to the individual teacher question catalogue to choose your five additional questions. Further instructions are included in the documents.
Questionnaire form delivery method
When you request your questionnaire, you will need to specify whether you would like your students to fill out paper questionnaire forms in class, or complete the questionnaire online (a particularly useful option for distance papers).
If using Otago inFORM, you will be asked to select your chosen distribution method at the checkout stage.
If you lodge your questionnaire request by sending in a paper form, you will need to fill in a different form depending on the delivery method you wish to choose. The two forms are labelled on the forms and resources page.
Confidentiality policy for individual teacher questionnaires
The results of your questionnaires will be confidential to you. The evaluation section of HEDC will not supply anyone else with copies or identifiable information from your questionnaire without your authorisation. The rare exception is when student questionnaires appear to have been altered inappropriately, in which case they cannot be processed and will be passed to the Head of HEDC and the relevant PVC for advice. Anonymised data may be used by the University.
How you use the information resulting from the questionnaire is a matter for you to decide. However, if you are a candidate for promotion, confirmation or appraisal, you will be expected to submit the Evaluations of Teaching: Summary Data reports with your application or report. HEDC staff are available to assist you with the interpretation of your results or to design a plan for improvement.
